I understand that the state of Tennessee issues licenses to health and wellness professionals authorizing them to analyze, assess, diagnose, evaluate, examine, advise, caution, counsel, guide, prescribe, recommend and suggest cures, prescribed drugs, interventions, remedies, and treatments to address what is wrong with them. I understand that services from Ashley Finley will refer me to a properly licenses professional if I need, or if I feel a need for a medical person to diagnose, treat, counsel, or cure me of anything.
I understand that biofeedback is not a substitute for regular medical care through any licensed doctor or medical person. I understand that Ashely Finley is not attempting to cure, diagnose, or treat any condition, diagnosis, or disorder.
I understand that Ashley is a certified Quantum Biofeedback technician and that she has been properly certified to help me make more informed decisions about my own life and healthcare. I also understand that biofeedback is intended to help me relax so that I can manage my pain and stress and improve the quality of my life.
Client confidentiality is of the most important to Ashley, but there are exceptions to confidentiality. Child or spousal abuse, alcohol or drug abuse, knowledge about the client breaking the law, threats of committing suicide, threats of doing violence toward others and/or killing other people. If there is any evidence or if client reports any of these things, Ashley will report these things to the proper authorities.
I understand that Ashley will keep all information she learns about being confidential unless I release her in writing or required by law to do so. I further understand that Ashley will not acknowledge my presence or discuss anything with me publicly unless I initiate the conversation and the topic of discussion.
I understand that my identity and information about me, whether I share it with Ashley or she discovers it on her own, will be held in the strictest confidence, except when released by me or specifically required by law. I have the right to waive this confidentiality agreement in whole or in part at any time. I also have the right to stop by feedback at any time.
I understand these products and services are nonrefundable.
